PLEASE READ THE FOLLOWING CAREFULLY:

By completing this form, you are requesting to surrender your dog into Alberta Homeward Hound Rescue Bureau's (AHHRB or Rescue) care. This is the first step in the intake process.

We understand that circumstances in life can change and you may no longer be able to care for your pet. Information provided to us about your dogs' personality and needs is a vital part of the process as we prepare him/ her for adoption into a new forever home.

​Once we receive the application, it will be reviewed and verified. A member of our intakes team will contact you once the initial review is completed to discuss the application. Once all of the necessary information is collected, a decision will be made if the rescue will be able to bring the dog into our care. There are a number of considerations that are taken into account when accepting an owner surrendered dog but the first priority is confirming that we have a foster home available that will best suit the needs of this dog.

Veterinary records must be submitted at time of surrender. We will not be able to accept any owner surrender dogs without their veterinary history. Confirmation from the vet clinic about transfer of information to AHHRB is acceptable.

    If your dog is not purebred, please indicate the cross type as well.
    Upon surrender, we will ask for your animal's full medical file to be submitted so that our veterinarians have the proper history and information to make the best decisions they can for this animal. By completing this agreement, you are agreeing to submit or transfer ownership of these records to Alberta Homeward Hound Rescue Bureau.
    If your dog is on medication, we ask that any remaining medication as well as dosage be submitted at time of surrender with them.
    playful, assertive, lazy, aggressive... fun loving, easy going... give us a good idea of who they are! This is important for us to make sure we have a foster home that will be the right match for them.

I(We) have not falsified my(our) responses and I(we) understand that my(our) application can be denied if it is determined that I(we) have not been truthful in this application.
